Our offices are open daily, Monday throught Friday, with the exception of holidays.
To schedule an appointment, please call (804) 746-3505 during office hours. Please
bring the following with you each time you visit our office: Your insurance card,
your driver's license (or other picture identification), and a list of your current
medications. If this is your first visit with us and you would like to pre-register;
the registration and patient information forms are located on this page under Patient
Forms. Just print the forms, fill them out and bring them with you to our office.
To protect your right to privacy, all information concerning your medical diagnosis
and treatment at Lee-Davis Medical Center remains confidential. We will not release
any information about you without your permission to do so.
